I hereby state that I have read and agree to the following

PAYMENT TERMS & CONDITIONS
Requests to cancel or transfer must be made in writing, email: info@ritesofpassageinstitute.org. Cancellations made more than 6 weeks before the start of a camp will be refunded fees, less a $150 administration fee. Cancellations made between 6 and 3 weeks before the start of the camp will be refunded fees less $550. Cancellations made less than 3 weeks before the start of the camp will not be refunded. In the event of needing to change a booking, it is possible to transfer to a future camp (if space is available) an administration fee of $550 will be applicable to all transfers. No refunds are given for no-shows. In the event of unforeseen circumstances, we reserve the right to change or cancel camps.

CONSENT
I being the parent/guardian of the child attending declare that I understand that the activities may involve running, jumping, fire and water as well as being in the bush, thus exposing my child to situations and physical activities not encountered in the classroom. I acknowledge that there will be every reasonable effort made to minimise exposure to known risks. However, all hazards and dangers associated with these activities cannot be foreseen or may be beyond the control of staff. I acknowledge that this camp will be held in a bush environment and while every effort is made to ensure the safety and well being of my child, there is always a possibility of sickness or injuries occurring. I acknowledge that there will be trained first aid staff on the camp and that for any serious medical issues that may arise, there is an evacuation procedure which includes any affected participant being transferred to the nearest hospital. In the event of an ambulance being required or extra medical fees arising, they will be the responsibility of the participant. I consent to receiving marketing communications from The Rites of Passage Institute. Any photographs taken at this event can be used by The Rites of Passage Institute.
